Setting up MySQL on a hosted server

Hey there. I am admin on a minecraft server and we have run into a problem with mysql. Im having a problem setting up plugins with the mysql the host provided us. I cant figure out what im doing wrong and im hoping someone here would be able to steer me in the right direction. Help would be very appreciated right now as we are close to finishing the setup of our server and we are keen to get it out into the public.
