Death screen not appearing

I can't seem to figure out the cause of the problem or fix it, basically when a player dies, the death screen with the 'Respawn' or 'Title Screen' buttons doesn't appear and the player just remains where they died. The items go all over the floor and the 'To return to death point use /back' message from essentials still appears but the player can just pick their items back up and be on their way.
Any Help is Much Appreciated
