Chat (Herochat) - Factions Error

I updated some plugins today and factions seems to be doing this with chat now (Look at attached image)
It double's the message and said the world your in which we dont want, i just need a fix for this.

My plugins i have are in the other screenshot, if you need any plugins cleared up just ask.
Im not sure if this is conficting, or what, but i really fix for the format to be
(Chat Channel) (Faction) ( Rank) (Name) (Message)

The newest factions update does not use [FACTION] setting anymore. Please refer to this page. http://dev.bukkit.org/bukkit-plugins/factions/files/38-1-7-5-beta/

 

PolarCraft I read that and tried using it, nothing seemed to change .
