I understand that isn't your issue but I can't look at your perms without pastebin being up... See the issue here? You need to tell us what plugins you are using because some have different formatting requirements than others! Here are some general rules I follow (Remember permission plugins read top to bottom and when they find what they need they stop reading the file): You need to put inheritance at the bottom of each group. You also do not need "default: false" on every group. You also need not put [] around your prefixes as your chat plugin should be the one handling that part.
