PermissionsEx Problem

I have a problem with permissionsEx, I made a permissions.yml file by editing it without using the commands, and it doesn't work. I change it around, add stuff ingame and it works but i need to use it with this here is the permissions.yml file at pastebin: http://pastebin.com/ShNxXN7Z

Issues:

• Your Admin and Head-Moderator had issues with there perms, that's all.
