Plugin Help PermissionsEx not working! Please help!

Okay, so permissionsex just broke about 10 mins ago. All I did when I went into my FTP is add 2 more ranks (Youtuber and Twitch). That is all I did, didnt go into any other plugins folders except the permissions.yml for pex. Now when I go ingame players name in the TAB list show up as example: '{permprefix}Notch{permsuffix}'
Also there are NO prefixes seen in chat and they were working fine before.
And lastly, no one has perms for anything. They can't do anything that requires perms.
I have had this issue before, a few days ago. I just did a rollback and it fixed it but I don't know what to do. I don't want to do another rollback.
