that means 1) The plugin that uses the command is broken from the start, and will always throw an internal error or 2) The permissions file that you have is broken - unparsable yaml format, or illogical structure to the permission plugin reading it - and a broken permission file ==> broken permission system ==> commands need to check if you have the permissions to use the command, but are not able to because there is nothing to check with, so it dies right there. And /pex means you have permissionsEx on the server /bpermissions would mean you are using bpermissions You cant have both - you can only have one permission manager system on a server at a time.
You should post a stacktrace of the error shown in console. That way we could find the exact issue instead of guessing randomly what the problem could be.
