Anyone Know Whats Causing This Error?

I keep getting this.
It will basically just happen again and again.

Most the time it don't do this though.
Something triggers it, and then it won't stop.

It don't always happen either.
So i am lost.

Can you find out what triggers it?

 

No its quite random.
Usually it is a strange logout, so i think it is our ban system.
However, once its triggered it can just happen randomly.

Really weird.
It don't always happen either, like it hasn't in the last 8 Hours.

 

If you don't mind me asking, is your ban system a public or private system?
I can take a look at the code if you want.
