I configurated my permissions file, checked in YML Parser and put it in, when I done, plugins like dynmap,LWC, Essentials,etc stopped working. Here, I remade your PermissionsEX.yml file for you. The problem was your spacing's were off, Make a back up of your PEX file encase the one I made does not work. Oh and I see you have Prefix's for your ranks make sure to get a chat plugin so your rank prefix's will show.
