   The death event should be helpful. As far as I can tell, it is called after they die, but I haven't used it in that manner yet.
   
   As far as given them an item, you can make the item (ItemStack), and then event.getPlayer().getInventory().addItem(item), where item is the name of your itemstack.

   You could change it so it drops it on respawn event and it should pick it up meaning you basically spawn with it.
