Custom chat format with Factions

If at all possible, this is how I would like to setup my general chat format for players talking in chat:
{S} {FACTION} {RANK} {NAME}: {MSG}

{S} The first letter of the world/map that the player is currently in, I'm using Multiverse-Portals to switch between two other worlds. eg. {S} when chatting in Survival world
{FACTION} Factions plugin: name of the faction the player is currently in
{RANK} Different colours for different ranks, I think PEX makes this happen automatically
{NAME} Player name

Does anyone have experience with something similar to this? Where do I even start? Which (chat) plugin should I use in combination with Factions? Or should I replace certain plugins?

I'm using PermissionsEX, ChatManager, Multiverse and Multiverse-Portals and Factions.

Essentials will support this, but your chat format will be so long it will clog up the chat log
