I'm having the same issue. When I hit 300 players the CPU usage will spike frequently and cause some lag. It was fine when I was using R4.0.
I think it works now, seems to have been a plugin conflict.
You will need to queue commands.

It's definitely a chunk sending conflict. Orebfuscator uses ChunkScrambler, and that must be conflicting with the way NoLagg sends chunks.
Permissions usually only lets you define two values, prefix and suffix.
I suggested that you put the suffix value directly in front of the display...
If you don't use suffixes, turn the suffix into a color code and insert the suffix before the player in the respective chat configuration plugin
Orebfuscator, but if you decide to use it be prepared to lose players that have terrible internet connections.
CraftProxy does not perform well on large servers. Do not use it if you have over 100 players online.
Disable both NoLagg and Orebfuscator. There may be a chunk sending conflict.
